Corporate Entry Form

Participants can run or walk the course and are encouraged to raise pledges to benefit Hasbro Children s and Bradley hospitals. ASICS will award running shoes to all scoring members of award winning teams in each division. Our goal is to increase our donations from previous years and to do this we need your help. So take the challenge and enter a team. Together we can make a difference. All necessary information to register your team is included in this package . CORPORATE TEAM GUIDELINES

• IMPORTANT: Team members can only score for one (1) division.
• All team members must be full-time employees working at least thirty-five hours per week.
• Men s team must consist of at least three (3) men. Women s team must consist of at least three (3) women.
• No mixed teams (a team with men and women).
• There is no limit on team size and you do not need both a men s and women s team to compete.
• Enter only one (1) team of unlimited size per division per company. Each division must use a separate team entry form.
• Teams may have both runners and walkers.
• Masters entrants may be members of an Open Division team but cannot score for both the Open and Masters Division.
• Teams from running related businesses, e.g. running shoe companies, will not be eligible for corporate team awards.
• A team member must be present at award ceremony to receive trophy.

HABRO YOUTH TEAM CHALLENGE

Any child or relative of an employee, as well as their friends, are welcomed to join the fun and run in the Youth Races. Employees are encouraged to sign up children as part of their corporate team's youth division. Organizations such as soccer teams are also welcome to enter a team. The team with the most children in their youth division and the team that raises the most money in pledges will each recive a prize from Hasbro.

• Any number of runners can be on a youth team
• All team members' individual entry forms must be submitted together wtih the team entry form.

ENTRY STEPS

1. Complete one (1) Team Entry form for each division; list the team members.
2. Each team member must fill out an individual entry form along with $15 entry fee (before 9/1), $17 (after 9/1)
3. Mail the team and all individual entry forms for the team members together in one (1) envelope postmarked no later than
   9/8/99 to:
